Request Eyeglasses
Organizations may request recycled eyeglasses from EYEsee for their planned overseas health missions by supplying EYEsee with the following information with their requests. Please make sure that all requests are made at least 3 months in advance of the planned health mission:
1. Information about the organization.
a. Name and address of the organizationb. Purpose of the organization
c. Website address, if available
d. Contact name, phone number, and email address
2. Information about the planned health mission
a. Country of planned health missionb. Dates of planned health mission
c. Beneficiaries of planned health mission
d. Plan for distributing the recycled eyeglasses at the health mission site
e. Quantity of recycled eyeglasses needed
f. Address where EYEsee will ship the eyeglasses
The completed form can be sent to glasses@eyeseemission.org. Organizations must agree to provide EYEsee a mission report within 30 days after completion of their overseas health mission. The report will detail how EYEsee’s recycled eyeglasses were distributed during the health mission and the beneficiaries who received the eyeglasses. The mission report must include photos showing the actual distribution of the eyeglasses at the site of the health mission.
